Bowling Green Physical Therapy LLC is a veteran-owned physical therapy practice serving South Central Kentucky with a mission to deliver elite-level care to everyday people.

We believe physical therapy should be personal, proactive, and performance-driven — not rushed or dictated by insurance companies.

Meet Tavis Ciochetty, DPT

Founder & Owner | Lead Physical Therapist | Veteran | Former Division I Athlete

Tavis Ciochetty, DPT, OCS is the Lead Physical Therapist and founder of Bowling Green Physical Therapy LLC. With over 8 years of experience across outpatient orthopedic practice and the U.S. Army, Tavis brings a rare blend of clinical expertise, athletic performance knowledge (as a previous D1 football player), and military discipline to every client he serves.

Tavis earned his Doctor of Physical Therapy degree from the University of Kentucky, where his doctoral research was published in the Journal of Strength and Conditioning Research. His academic foundation, paired with real-world experience, shapes his performance-driven approach to rehabilitation and injury prevention.

After direct commissioning, Tavis served 5.5 years on Active Duty as an Army Physical Therapist, practicing at Winn Army Community Hospital and Hawks Troop Medical Clinic at Fort Stewart, Georgia, before serving as an Infantry Brigade Combat Team Physical Therapist with the 101st Airborne (Air Assault) Division at Fort Campbell. He later continued his service as a civilian physical therapist at Fort Campbell until 2025.

During his military career, Tavis earned his Air Assault Wings, achieved his Orthopedic Clinical Specialist (OCS) Board Certification, and was awarded the Expert Field Medical Badge (EFMB) — one of the most challenging and respected achievements in the Army Medical Department, earned by fewer than 25% of candidates.

Today, Tavis applies the same standards he used caring for soldiers and tactical professionals to active adults, athletes, first responders, and employers throughout South Central Kentucky. His care philosophy emphasizes movement quality, strength, durability, and long-term performance, not just symptom relief.

A Bowling Green native, both a graduate of Greenwood Highschool and Western Kentucky University, Tavis returned home in 2025, where he lives with his wife, Hallie, and their four children — Everly, Boone, Amelia, and TJ. He is proud to operate a veteran-owned physical therapy practice serving the community he calls home.
